By signing this form, I confirm that I voluntarily agree to participate in the 60 Women 60 Years project.

I understand that my contribution may be collected through an interview or written submission. I understand that my contribution may be edited, condensed and arranged for clarity, length and consistency while seeking to preserve the meaning and spirit of my story.

I give permission for the project organisers, author, publisher and their authorised representatives to use my submitted information, interview material and approved profile for the purposes of creating, publishing, distributing and promoting 60 Women 60 Years. This may include use in:

• the printed and/or digital edition of the book;
• websites and digital platforms associated with the project;
• social media;
• press, publicity and media activity;
• launch events, exhibitions and presentations;
• promotional materials relating to the book; and
• future promotion of the book and the 60 Women 60 Years project.

Commercial Publication & Social Impact. I understand that 60 Women 60 Years is a published work and that the book and associated products may be sold commercially. I understand that my participation does not create an entitlement to royalties, fees or other payment unless separately agreed in writing.

I also understand that a percentage of the proceeds from the 60 Women 60 Years project will be used to support charitable activities and initiatives designed to create positive social impact in Guyana. The specific activities, organisations or initiatives supported may be determined by the project team and may vary over time.

Through the project, 60 Women 60 Years aims not only to celebrate and preserve the stories of Guyanese women, but also to contribute positively to the country and communities whose stories inspired it.

We want every woman featured in 60 Women 60 Years to feel that her story has been represented with care.

If you choose to review your profile: your draft profile will be sent to the email address you have provided. You will have five (5) working days from the date the profile is sent to notify the project team in writing of any factual inaccuracies, significant misrepresentation or other objections to publication. If we do not receive a response within those five working days, you will be deemed to have approved the profile and consented to its publication in the form provided to you, subject to final editorial changes that do not materially alter the meaning of your contribution.

Any requested amendments received within the five-working-day period will be considered by the editorial team. The review process is intended primarily to correct factual inaccuracies or significant misrepresentation rather than to rewrite the profile. Final editorial decisions relating to style, length, layout, design and publication remain with the 60 Women 60 Years editorial team.

The personal contact information collected through this form will be used to administer the 60 Women 60 Years project and will not be published unless I have specifically agreed to its publication.

Information intended for publication — including my name, biography, heritage information, interview or written contributions and photographs — may become publicly available once the book or related promotional materials have been published.

I understand that some questions about heritage or ethnicity are optional and I do not have to provide this information if I do not wish to.

I understand that once material has been published or distributed, it may not always be possible to remove it from copies already printed, distributed, downloaded, shared or otherwise made publicly available.

If I have questions about how my information will be used, I can contact the project team before signing this form.
